Workers’ Compensation

Care is available for work-related injuries and return-to-work goals. When you call to schedule, have your claim number and employer information ready so we can confirm authorization and coordinate with your adjuster or case manager as needed.

Cash-pay options

If you do not have insurance or choose not to use your policy, cash-pay options are available. Contact the clinic to discuss pricing, payment arrangements, and scheduling. We are happy to provide a receipt you can submit for out-of-network reimbursement if your plan allows it.

Referrals & getting started

Whether you need a physician referral depends on your insurance plan and state regulations. Some plans allow direct access to physical therapy, while others require a referral or authorization before your first visit. When you call to schedule, have the following information ready:

  • Insurance card with subscriber ID and group number
  • Date of birth of the subscriber
  • Referring provider's name and contact information, if applicable
  • Any authorization or claim numbers, if you have them
